Increased cancer risk in patients with IgG4-related disease in a nationwide South Korean cohort, 2012-2021.

This nationwide population-based cohort study evaluated cancer risk in 2,150 patients with immunoglobulin G4-related disease (IgG4-RD) using the Korean National Health Insurance Service database.

Patients showed a significantly increased overall cancer risk (SIR 4.12), including:

  • Solid tumors (SIR 3.33)
  • Hematologic malignancies (SIR 15.31)

The highest risks among solid tumors were for:

  • Pancreatic cancer
  • Central nervous system cancer
  • Biliary tract cancer

The most common hematologic cancers were:

  • Myelodysplastic syndrome
  • Non-Hodgkin lymphoma

Cancer risk was notably higher in females and peaked within the first year after IgG4-RD diagnosis.

These findings highlight the need for vigilant malignancy surveillance, especially early after diagnosis.
