Efficacy of endoscopic therapy in patients with T1b gastric cancer and construction of a prognostic prediction model: a retrospective cohort study and multicenter validation study.

Journal: Surgical endoscopy

This study compared long-term survival outcomes among T1b gastric cancer patients treated with endoscopic therapy, gastrectomy, or chemoradiotherapy using data from the SEER database (2010-2020) and a Chinese multicenter cohort.

Key results include:

  • No significant difference in overall or cancer-specific survival between gastrectomy and endoscopic treatment.
  • Chemoradiotherapy was associated with poorer outcomes.

A prognostic model was developed using Lasso regression, incorporating:

  • Race
  • Gender
  • Age
  • Histologic grade
  • Tumor size
  • Treatment modality

This model showed good predictive accuracy for overall survival.

The findings support endoscopic therapy as an effective alternative to gastrectomy for T1b gastric cancer patients.
